Welcome to the Registrar's Office

Hello Families,

I register all new students from Kindergarten through 12th grade by appointment, as well as handle withdrawals and changes of addresses. If you have a child entering elementary school and are unsure of the school your child should attend, I can provide that information.

To register a new student:

  • Review the Registration Packet and Instruction links below regarding the registration procedures and documents required for new student registration.

  • Contact me to schedule a registration appointment. All registration appointments are held in person and by appointment only in the District Office. No Walk-In's Accepted. If you walk-in, we will make an appointment for another day.

  • Please bring with you all the necessary documents to register your child. The completed registration packet; Proof of Age: birth certificate or passport; recent school physical and immunization records; recent report card from prior school; if any, custodial documentation and (3) current proofs of residency. Registration is not complete without these documents. All physicals and immunizations must be reviewed by our school nurses in order to clear your child(ren) for entry into the schools.

  • When you come to the District Office for your registration appointment, please remember to bring a photo ID for the security checkpoint.

Screening Appointments: All new students in all grades will be screened.

  • As per the New York State requirements, all new students from out-of-state or private schools or from out of the country will be screened in the areas of health, motor skills, language skills and cognitive concepts. These short screeners are administered by staff members from your child’s school during regular school hours.

  • If you are registering your incoming kindergartener, screening appointment's are scheduled at the time of registration, which are held on specific dates.

Change of Address

  • If you have moved within District, please provide me with (3) current proofs of residency in PDF format. All proofs must provide your name, new address, company name, address and a current date. Please do not send photos or screenshots as they are not always clear to read. Once I have that, I will update in the system.

Location
Our office is located at the District Office on the John Jay High School campus, 60 North Salem Road, Route 121, Cross River, NY 10518. 

Hours
Appointment hours are Monday through Friday, from 9:00 am to 3:00 pm during the school year and 9:00 am to 2:00 pm during the summer months and non-session days. 

If you are not the parent/legal guardian of the student

If you are not the parent/legal guardian of the child/ren being registered, the Parent and Guardian Affidavits must be completed. Please refer to the Katonah-Lewisboro Registration Packet Instructions for additional information: Parent and Guardian Affidavits

Withdrawing a student

We are required to have parents or guardians complete a Withdrawal Form if they are changing schools or moving out of District. Please complete each section with the grade and last day your student was in school in the withdrawal form below and send it to Victoria Friedman (Email Victoria Friedman). Please complete the Release of Records with your student's new school that they will be attending. When you email it, please send it as a PDF format as photos and screenshots are not clear to read. If you require further assistance with the withdrawal form, please contact Victoria Friedman.
